Advance Care Directives
Make your wishes known about your future health care and lifestyle.
An Advance Care Directive records your wishes and values about future health care, where you live and personal matters. You can also appoint Substitute Decision-Makers to decide for you if you can't.
It can cover
Medical treatment and end-of-life care, including treatment you would refuse
Where you live, including residential and aged care
Personal matters such as diet, dress and religious or cultural practices
Advance Care Directives replaced the old Enduring Power of Guardianship, Medical Power of Attorney and Anticipatory Direction. Those documents remain valid if they were made before 1 July 2014.
The directive must be completed on the approved form and signed in front of an authorised witness. We can help you complete it and witness it for you.
Many clients make one at the same time as their Will and Enduring Power of Attorney, so everything is in order.
